Abstract

Introduction: Calcifying Cystic Odontogenic Tumour (CCOT) is a rare, developmental lesion accounting for 6% of central odontogenic tumors. Classified by the World Health Organization of Head and Neck Tumours as an odontogenic tumour, a spectrum of histopathological features can be seen including cystic, solid (neoplastic) and aggressive (malignant) variants. Case Report A fit and healthy 26 year old female was referred from her GDP to the Oral Medicine Department of Glasgow Dental Hospital and School complaining of a sudden onset, painless lump on the gum in the left upper jaw noted at late stage of pregnancy. An onward referral to Oral Surgery was made for investigation and treatment. Plain imaging revealed a 2cm, corticated unilocular radiolucency in the left maxilla with root displacement and CBCT and tissue sample was carried out. Discussion: The CBCT showed a well-defined, corticated, buccally expansile radiolucency in the left maxilla with perforation and thinning of the buccal cortex. Displacement and rotation of teeth and resorption of root surfaces was also seen. Differential diagnosis included an ameloblastoma. Based on the clinical and radiographic findings a tissue sample was taken to aid diagnosis. Diagnosis of CCOT was made and the appropriate surgical management undertaken. The communication between the oral surgery team, the radiologist and pathologist was key to establishing the correct diagnosis and ensuring appropriate surgical management. Conclusion: This case highlights the importance of clinical suspicion, tissue diagnosis and 3D imaging when root displacement or resorption is seen, underpinning the importance of multidisciplinary care.
